Clinics & Services
The practice works under a General Medical Services Contract for the provision of healthcare services. The defines certain minimum services that must be provided, as well a number of other optional services. Within these services, we also hold a number of specific clinics within the practice to best support our patients.
Essential Services
Core Services
All practices must provide core services during core hours. These services must be appropriate to meet the reasonable needs of patients.
Essential services are those required for the management of registered patients and temporary residents who are, or believe themselves to be:
ill with conditions from which recovery is generally expected;
terminally ill;
suffering from chronic disease,
which are delivered in the manner determined by practice in discussion with the patient.
Cervical Screening Services
We offer this service to provide necessary information and advice to assist women who are identified by NHS England as recommended for a cervical screening test in making an informed decision as to their participation in the NHS Cervical Screening Programme. We will carry out cervical screening tests according to national guidelines and assist with call and recall programmes. Child Health Surveillance Services
We offer this service to proactively monitor the health, well-being and physical, mental and social development of children up to age five with a view to detecting any deviations from normal development. Children will be invited for reviews at certain ages and we encourage parents to attend for these.
Contraceptive Services
We can help with all of the following:
- advice about the full range of contraceptive methods, including where appropriate, a medical examination of patients seeking such advice;
- treatment for patients with a range of contraceptive substances and appliances (excluding the fitting and implanting of intrauterine devices and implants;
- advice about emergency contraception and, where appropriate, the supplying or prescribing of emergency hormonal contraception;
- advice and referral in cases of unplanned pregnancy including advice about the availability of free pregnancy testing in the area;
- initial advice about sexual health promotion and sexually transmitted infections;
- referral as necessary to specialist sexual health services, including tests for sexually transmitted infections.
Maternity Medical Services
We offer the following services in relation to pregnancy and child birth:
- routine care throughout the pregnancy and after birth;
- an invitation to attend a maternal postnatal consultation within six to eight weeks after the birth;
- care and support for patients whose pregnancy has terminated as a result of miscarriage or abortion.
We do not provide care during labour and this will be offered by the hospital team.
Vaccination and Immunisation Services
We will offer and provide routine vaccinations and immunisations for the following diseases.
Children
All routine vaccinations to protect against:
- diphtheria;
- tetanus;
- pertussis (whooping cough;
- polio;
- haemophilus influenzae type B (Hib);
- hepatitis B;
- meningococcal group b;
- rotavirus gastroenteritis;
- pneumococcal infection;
- meningitis type c;
- measles;
- mumps;
- rubella;
Some vaccines are combined as a single dose, for example measles, mumps and rubella is a single injection against all three diseases.
Adults
The following are offered depending on age:
- pneumococcal infection (age 2-64 in a clinical risk group and everyone 65 and over);
- shingles (age 70 and over);
- measles, mumps and rubella (age 16 and over where incomplete vaccination in childhood);
- pertussis (for pregnant women from 16 weeks of pregnancy).
Optional Services
We do not provide the following optional services. Greater Manchester Integrated Care Partnership is responsible for ensuring our patients can access these services.

Enhanced Services
These are also optional for practices to provide. We have chosen to provide the following enhanced services.
Learning Disabilities Health Check Scheme
We will keep a register of all patient aged 14 and over who are known to the local authority social services department as having a learning disability, and offer and provide an annual health check tailored to their needs.
Network Contract DES
We have signed up to this directed enhanced service. It requires practices to work collaboratively to best support the needs of their local practice population. We are working with other local practices under the banner of Salford Digital First Primary Care Network (PCN). The main service currently provided by our PCN is evening and weekend appointments. Seasonal Influenza Vaccination
We will identify and invite eligible patients to receive an appropriate vaccination against ‘flu during the campaign season which is usually from September to March. The eligibility criteria and vaccinations to be used are decided nationally and not by the practice.

PCN Dietician and Social Prescriber
Our Primary Care Network (PCN) offers a Dietitian (Chloe) and a Social Prescriber (Zoe) who support people living with type 2 diabetes, pre-diabetes (also called non-diabetic hyperglycaemia) and obesity. They can help you to take control of your health and wellbeing and make positive changes to your lifestyle.
They offer one-to-one sessions at a local practice, group education sessions in a community venue and diabetes education drop-in sessions. They also regularly attend community events to offer information education around healthier living. Chloe and Zoe have teamed up with MCR Active to include an exercise element alongside their own education offer.

Chloe has worked as a registered dietitian for over 10 years with lots of experience working in hospitals and GP practices.
As a registered dietitian Chloe’s goal is to translate nutritional science into practical and easy to understand information. Chloe is extremely passionate about the role nutrition plays in allowing people to live healthier and happier lives. She uses a holistic approach and tailors her recommendations to accommodate people’s individual needs.
Zoe Austin - Social Prescriber (07835 261450 zoe.austin7@nhs.net)
Zoe has worked as part of the NHS for nearly 10 years and is a member of the National Association for Link Workers. She has gained a lot of her experience working within community rehabilitation services, helping to increase the independence of people living with a wide range of health conditions. Zoe is passionate about supporting people who may be struggling to adapt a healthier lifestyle, helping to increase their health confidence and outcomes through a person-centred approach.
